Registration for the 2008-2009 school year will be

– from the previous school year, showing
promotion
3. * Copy of Birth Certificate 4. * Copy of Social Security Card 5. * Immunization / Ear, Eye, & Dental – Parents have 30 days to have Immunization records transferred to Georgia Form #3231 and 120 days to have Ear, Eye, & Dental transferred to Georgia Form #3300. This can be done at the Columbia County Health Department or with your personal physician. 6. Gifted or Special Education Information (If Applicable) 7. Custody/Guardianship Papers (If Applicable) |
