Registration Requirements

1. Withdrawal Form - only if withdrawing
during the school year.
2. Last report card
3. Birth certificate
4. Social security card
5. Immunization record needed at time of enrollment (will need to be on Georgia form #3231 within 30 days. Ear, Eye and Dental form #3300 is due within 120 days)
6. Proof of residency (power bill, etc.)
7. Custody/Guardianship paper (if applicable)
